Please complete and include this form with your ironing, laundry or dry cleaning if:

1. You would prefer to have your shirts/blouses returned folded rather than hung.
   
   
2. You want to bring our attention to a particular stain.
   
   
3. You are concerned about an item, it may have been particularly expensive, it may be particularly delicate or just a favourite of yours.
   
   
4. We should in any event be informed of any single item that has a replacement price in excess of £250. If we are not informed and any damage does result then sorry, we can't be held responsible.
   
   

Certain instructions may require us needing more time than our normal schedule, we will let you know if this is the case.
